I’m sure it must, just not under pressure and not enough, to not burn the impeller, if it was running! I’ve seen it written somewhere that running a motor dry will burn an impeller up in like 30/45 seconds?
Yamaha says not to run the engine on the flush adapter because they do not want you to potentially burn up the water pump.
Yamaha use to say it was okay to run the engine at low rpms while using the flush adapter.
if you keep the engine at idle only, nothing bad will happen if you run the engine on the adapter, because enough water gets to the impeller to keep it cool.
running the motor while flushing is MUCH better than cold flushing. you are opening the thermostats, and heating the engine, providing for a more thorough flush. As for the motor not peeing on the adapter, you have an issue somewhere, because that motor should pee on the flush adapter, you are still pressurizing the system, but I have seen a few motors that just wont pee on the adapter. also a lot of the 200-250 4 stroke Yamahas don’t like to pee on the muff adapter, but will pee fine in a drum.
the water from up top does not go through the impeller before exiting the prop, but water will back flow down to the water pump and keep it cool. the waterpump is incoming water and its completely different passages than the exhaust

Our 250s pee when the hose is hooked up. Sometimes you gotta mess with the whole for the water to come out. I’m not sure why.

My F250 pees when on the flushing attachment. It will overheat if I try to run on the muffs. Just can’t get enough water from the garden hose. I have hooked up two hoses multiple times one to the foot with the muffs, one to the flushing connection. I figure this way it is getting enough water to keep it from overheating the head and athe foot is getting enough water not to run the impeller dry. Any harm in doing this?
